Welcome to our Online Data Submission Tool. All new customers must submit their information to us using this tool in advance of using our services.
This tool is intended for new customers. If you have already submitted this or other customer forms in the past, please contact Tucker Pup's if you need to add or change any information.
Please note:
- We don't automatically call you upon receiving these forms. You must still call us or make an online request in order to use any of our services.
- Because of the time and detail involved, it is better if you fill out these forms before you arrive.
- This tool is different than our Request More Information form, which some people fill out to express their interest in using our services.
Before you begin, please have ready:
- Work and mobile phone numbers for any other person on your account, such as your spouse
- Your dog's vaccinations, because you will need to enter the expiration dates. You can typically find these expiration dates on your most recent vet receipt, or you have have your vet fax a document to you that shows them.
- Phone numbers for any emergency contacts
