Tucker Pup's requires that you submit signed copies of the following forms before we can provide any services for your dog.

You can fill out these forms directly on the screen, then click on File | Print in your browser to print. You may also fill out any form with a pen. Please submit them to Tucker Pup's via fax at 312-276-8588, in person, or via mail.



IMPORTANT DOWNLOADING INSTRUCTIONS:

Because the files below are fairly large, it is typically best to open them as follows:
  • Be sure you have Adobe Acrobat Reader installed on your computer. Get it here if necessary.
  • Right-click on a link below
  • Click "Save Link As..." or "Save Target As..." (depending on your browser)
  • Save the file to your computer
  • Repeat for each file below
  • Open the files directly from your computer, typically by simply double-clicking them from wherever you saved them.

Otherwise, you can simply click on each link to open the corresponding file in your browser, although your screen may go white for a while while it downloads, and may time-out.
